On one side is imprinted a picture of Rabbi Yeshaya of Krestir and the form of his signature and on the other side the amulet and the inscription: "lesova velo lerazon "Bnei Chaie Mazonie".
The pleced in matching case with a page on which is a picture of the Zion of the Rabbi of Krestir Zia and the amulet letters of the three psukim.
