Seder Tikun Leil Shavuos and Hoshanah Rabba, “According to the seder printed in the sefer Shnei Luchos HaBris (SHLoH) and the Kavanos of the ARIZAL. Printed in large vowelized letters “with all the advantages of the Slavitta edition”.
Although this edition says that it is based on the Slavitta edition and indeed closely resembles the Slavitta format, it is unclear where this particular edition was printed or who the publishers were.
