"תיקוני הזוהר" שחבר התנא האלוקי רבי שמעון בר יוחאי. עם מראה מקומות מספר חמדת ימים. שאלוניקי, תקל"ד[1774], בדפוס רפאל יאודה קלעי ומרדכי נחמן, "במקור", 5 , קנ"ח ד'; בעותק שלפנינו חסר דף אחרון. 4 דף שאחרי השער הם מפתחות לספר.
בדף הפורזץ, בשער ובדף אחריו חתימות ורישומים עתיקים בכ"י ספרדי לא מפוענח.
כריכה עתיקה אולי מקורית?, נייר לבן עבה, כתמים, מעט קרעים וחסרים, באופן כללי מצב טוב – טוב מאוד.
