דף בכתיבה מרובעת ומנוקדת, תפילת "בריך שמיה" להוצאת ספר תורה, ותפילת "ובנוחה" להכנסת ספר תורה. [הונגריה], תרכ"ח [1868].
בשולי הדף, קולופון הכותב והנודב, רבי מאיר שיק מסֶמנִיץ: "ז"נ [זאת נדבת] אני הק' מאיר שיעק מילידי ק"ק סעמניטץ יע"א, בעזה"י היום יום ה' פ' ויקרא שנת תרכ"ח".
הכותב והמקדיש: רבי מאיר שיעק מסעמניץ, היה בבחרותו מתלמידי הגאון רבי משה שיף אב"ד רגנסדורף (נפטר תרט"ז) - ראה חומר מצורף. לא ידוע לנו היכן הוא התגורר אחר כך. מסתבר שרבי מאיר כתב את הדף לצורך תליה בבית הכנסת בקהילה בה הוא התגורר.
