חתימה עתיקה בדף הראשון.
עותק חסר ופגום. ב-ג, ט, יב-יו, כא-כד, כו-כז, כט-לב, לז-נב, נז-צו, צח-צט, קב-קג, קה-קח, [8] דף. חסרים 32 דפים: א, ד-ח, י-יא, יז-כ, כה, כח, לג-לו, נג-נו, צז, ק-קא, קד, ו-[7] דפים אחרונים (חלק מהדפים האחרונים הושלמו בכתב-יד על פני 5 עמודים. 18 ס"מ. מצב בינוני-גרוע. כתמים, בהם כתמי רטיבות, ובלאי. עקבות רטיבות עם סימני עובש ופטריה. קרעים חסרים רבים, עם פגיעות רבות בטקסט, משוקמים בחלקם בהדבקות נייר. דפים וקונטרסים רופפים ומנותקים. חיתוך דפים עם פגיעות בכותרות הדפים. ללא כריכה.
