As attested by the rebbe's personal attendant, the Rebbe used this Seder Havdalah during the years 2000 thru 2005.
Bound in leather and embossed with his name.
Many names are handwritten.
it was known the Rebbe would allow people to write their name and their mother's name along with personal requests and he would have them in mind while reciting these prayers.
